Citizenship and national ID data. Dual citizens must disclose ALL nationalities, even unused or expired ones.

What this section asks

   Nationality (as shown on passport)

2. 2

   Do you hold or have you ever held any other nationality?

3. 3

   National Identification Number

   From your ID card or civil registry — NOT passport number.

4. 4

   U.S. Social Security Number

   Only if ever issued one. Otherwise 'Does Not Apply'.

5. 5

   U.S. Taxpayer ID (ITIN)

   Only if the IRS issued one.

Where people get it wrong

• → Dual citizenship is not a problem — hiding it is. Includes nationality by descent or marriage.
• → National ID is NOT your passport number. It's the internal civil-registry number; 'Does Not Apply' is common.
