Official CEAC section: Personal Information 1: Name, Date of Birth

Your name and date of birth must match your passport EXACTLY. Any mismatch is the #1 avoidable reason for visa delays and administrative processing.

What this section asks

   Surnames / Family Names (exactly as in passport)

   Letter-for-letter from the passport. No commas, no prefixes.

   Given Names (first + middle, exactly as in passport)

   Full Name in Native Alphabet

   If your native alphabet is Latin, 'Does Not Apply'.

   Have you used any other names (maiden, professional, religious)?

   Answer YES if any other name appears on any document or published work.

   Date of Birth

   City of Birth

   As named AT THE TIME of your birth.

   State/Province of Birth

Where people get it wrong

• → Copy your name letter-for-letter from the passport's machine-readable zone (the <<< line), not from the printed name page.
• → If you use a different name on documents or professionally, that goes in 'Other Names Used' — omitting known aliases is a misrepresentation risk.
• → CEAC times out after ~1 minute per page. Draft your answers here first, then paste them in.
